ABSTRACT

This article introduced the background, significance, working mode, talent training, qualification certification and clinical practice of oncology genetic nurses, providing the analysis of oncology genetic nurse development opportunities and challenges in our country. Proposals were put forward that China should draw lessons from successful experience of the development of oncology genetic nurses, strengthen the genetic nursing ability of nurses, establish oncology genetic nurse training mode, improve the post-setting and operation mechanism to meet the needs of hereditary oncology nursing management.

ABSTRACT

Objective@#To estimate the detection rate of low hemoglobin among primary and middle school students in Ningxia, and to explore association between physical growth indicators with level of hemoglobin in different economic zones from both the student and school levels, and to provide an effective basis for anemia prevention of primary and secondary school students.@*Methods@#The stratified random cluster sampling method was used to measure 5 447 primary and middle school students physical growth indicators and hemoglobin values. Prevalence of low hemoglobin was described and its association with physical growth indicators were analyzed through HLM models.@*Results@#The detection rate of low hemoglobin among primary and middle school students was 12.7%, of which 13.0% were boys and 12.4% were girls, without significant gender differences(χ2=0.43, P>0.05). Low hemoglobin in rural areas was higher than urban areas, as well as in the lowage group which was 18.5%, 12.6%, 8.3%, 11.5% in the 7, 9, 12, and 14-year-old groups, respectively, and the differences between the groups were statistically significant(χ2=43.49, 68.13, P<0.05). The wasting group had the highest detection rate of low hemoglobin, and low hemoglobin in obesity and overweight was lower than that of the normal group, with significant differences between groups(Z=2.08, P<0.05). Positive association between height and weight with hemoglobin was observed(γ 30 =0.13, γ 40=0.06), and the association of hip circumference with hemoglobin was negative(γ 50=-0.02). Body weight had a significant associated with the hemoglobin value in different economic film intervals(P<0.05).@*Conclusion@#The detection rate of low hemoglobin is relatively high among primary and middle school students in rural areas, low-age groups, and wasting groups in Ningxia. The differences between school groups are significant, and it is necessary to mine school-level variables.

ABSTRACT

Objective@#To understand the vitamin D nutritional status and associated factors of children and adolescents aged 6-18 in Yinchuan, so as to provide theoretical basis for improving the vitamin D malnutrition status of children and adolescents.@*Methods@#Using stratified cluster sampling method, 1 615 children and adolescents from 36 classes in 2 primary schools, junior high schools and high schools were randomly selected from Yinchuan, Ningxia. Venous blood sampling,physical examinations and questionnaires were conducted.@*Results@#The vitamin D deficiency rate and appropriate rate for students were 66.9% and 33.1%, respectively. There was a statistically significant difference in vitamin D deficiency among different age groups(χ2=25.65, P<0.05). The 6-year-old group had the highest(79.2%), followed by the 13-year-old group (76.6%), and the 12-year-old group (75.3%). The results of binary Logistic regression showed that the factors influencing the vitamin D nutritional status of children and adolescents included age(OR=0.95), daily outdoor exercise time(OR=0.49), exclusive breastfeeding (OR=0.63) and mother’s education(OR=0.73, 0.43). The decision tree QUEST and Logistic regression combination model results show that the key factors affecting vitamin D nutrition status include daily outdoor exercise time, age, mother’s education, BMI, and interaction between outdoor exercise time and age, interaction between outdoor exercise time and mother’s education, interaction between age and mother’s education, interaction between exclusive breastfeeding and BMI.@*Conclusion@#The detection rate of vitamin D deficiency in children and adolescents aged 6-18 in Yinchuan is high, factors associated with vitamin D nutritional status includes daily outdoor exercise time, age, mother’s education, BMI, and interaction between outdoor exercise time and age, interaction between outdoor exercise time and mother’s education, interaction between age and mother’s education, interaction between exclusive breastfeeding and BMI.

ABSTRACT

Objective:To investigate the effect of multidisciplinary team-based precise nursing on fatigue-pain-sleeping disorder symptom cluster of lung cancer patients received chemotherapy.Methods:A total of 110 lung cancer patients who received chemotherapy were randomly divided into study group (55 cases) and control group (55 cases) . The control group received routine nursing, while the study group carried out multidisciplinary team-based precise nursing. The degree of fatigue, pain and sleeping disorder was assessed by Multidimensional Fatigue Inventory-20 (MFI-20), Brief Pain Inventory-Chinese version (BPI-C) and Pittsburgh Sleep Quality Index (PSQI), respectively.Results:Before intervention, the scores of fatigue, pain and sleep quality showed no significant difference ( P>0.05). After intervention, the scores of physical fatigue, psychological fatigue, mental fatigue and total fatigue in MFI-20 were 17.78±3.96, 8.02±1.58, 10.19±2.01, 35.98±4.96 in the study group; the scores of pain right now, pain at least in the 24 hours, pain at worst in the 24 hours and pain on the average in BPI-C were 3.76±0.53, 2.15±0.39, 4.11±0.77, (2.72±0.43) in the study group; the scores of the habitual sleep efficiency, sleep latency, subjective sleep quality, sleep duration, sleep disturbances, daytime dysfunction, use of sleeping medication scores and total scores in PSQI were 0.96±0.20, 1.66±0.27, 1.74±0.36, 1.58±0.22, 1.12±0.21, 1.70±0.30, 1.45±0.28, 10.20±0.61 in the study group. However, those scores were 18.84±2.99, 9.15±1.95, 11.06±3.71, 39.05±5.28 and 5.42±1.18, 3.58±0.59, 6.18±1.44, 3.85±0.78 and 1.12±0.19, 1.74±0.27, 1.91±0.25, 1.66±0.46, 1.33±0.32, 1.75±0.29, 1.33±0.42, 10.85±0.70 in the control group. The scores of psychological fatigue, total fatigue, pain right now, pain at least in the 24 hours, pain at worst in the 24 hours, pain on the average and habitual sleep efficiency, subjective sleep quality, sleep disturbances and total PSQI scores were all significantly decreased in the study group compared to the control group ( t values were 2.815-14.873, P<0.01). Conclusion:Multidisciplinary team-based precise nursing can alleviate fatigue-pain-sleeping disorder symptom cluster of lung cancer patients during received chemotherapy.

ABSTRACT

Objective To investigate the correlation between diabetes distress and social support of elderly patients with diabetes mellitus. Methods Three instruments,including the general information questionnaire,the diabetes distress scale (DDS)and the social support rating scale (SSRS)were used to investigate the correlation between diabetes distress and social support of 95 elderly patients with diabetes mellitus from the endocrinology department of two tertiary level first-class comprehensive hospitals from at Novenber 2017.Results The total scores of diabetes distress are (32.7±11.5),at the level of mild distress.The proportion of moderate/severe level is 31.6%(30/95).The diamensions of "routine life distress" and "emotional distress" are most prominent;Social support overall and all diamension scores are lower than the model (all P<0.05);There is a significantly negative correlation between social support and diabetes distress overall,physical distress,routine life distress and interpersonal distress (all P<0.05). Conclusions The elderly patients with diabetes mellitus have different levels of diabetes distress and low level of social support.There is a negative correlation between diabetes distress and social support.We should strengthen the evaluation of diabetes distress and social support of the elderly patients with diabetes mellitus in clinical work,and take individualized interventions to improve the level of social support,in order to decrease diabetes distress and improve the quality of life.

ABSTRACT

Objective To investigate the antimicrobial resistance proifle of Shigella flexneri in Shanghai from 2010 to 2014 and examine the mechanism of fluoroquinolone resistance.Methods Kirby-Bauer method was used to determine the susceptibility of the S. flexneri strains to 14 antibiotics. The minimum inhibitory concentration (MIC) of ciprofloxacin was tested by E-test. Mutations within quinolone resistance determining regions (QRDR) of gyrA and parC and plasmid-mediated quinolone resistance (PMQR) genes,qnrA,qnrB,qnrS andaac(6')-Ib-crwere identified by polymerase chain reaction (PCR). All the products were subjected to sequencing analysis.Results More than 90 % of the 139S. flexneri isolates were resistant to ampicillin, streptomycin, tetracycline, and nalidixic acid, 40.3 % to ciprofloxacin and 30.2 % to cefepime, respectively. Genetic mutation of gyrA and parC was found in 98.6 % and 97.8 % of the strains, respectively. Three point mutations (Ser83, Asp87 and His211) were detected in gene gyrA and one point mutation (Ser80) was found in in gene parC. Plasmid-mediated resistant gene qnrS was found in 9 strains and aac(6')-Ib-cr in 6 strains.Conclusions The antibiotic resistance of S. flexneri is serious in Shanghai. The mutation rate within QRDR is high. Point mutation in Asp87 of gyrA is the main mechanism of quinolone resistance. The plasmid-mediated quinolone-resistant genes also play an important supplementary role.

ABSTRACT

Objective To investigate the antimicrobial characteristics of shigella flexneri and to analyze the correlation between acquired resistance genes and mobile genetic elements.Methods 139 strains of shigella flexneri collected from each district of Centers for Disease Control and Prevention in Shanghai from 2010 to 2014 were recovered.The K-B method was used to determine the susceptibility of the strains to 13 antibiotics.And then 17 kinds of acquired resistance genes to β-lactams, sulfonamides, aminoglycosides and 7 kinds of genetic markers of mobile genetic elements were analyzed by PCR.Index cluster analysis was performed to explore the correlation between them.Results Among 139 strains of Shigella flexneri,3 kinds of genetic markers of mobile genetic elements,ISEcp1,intI1 and trbC,3 kinds of acquired β-lactam-resistance genes,CTX-M,OXA and TEM,2 kinds of acquired aminoglycoside-resistance genes,ant(3")-I and aac(6′)-Ib, 1 kind of overlapping gene of quaternary ammonium disinfectant and sulfonamides, qacEΔ1-sull and 1 kind of sulfamethoxazole/trimethoprim-resistant gene, dfrA1 were detected.The resistence genes,OXA,ant(3")-I and drfA1 were highly related with each other,which were mediated by Class 1 integron.TEM,qacEΔl-sull and aac(6′)-Ib were highly related with each other,which were mediated by trbC.Conclusion Acquired multidrug resistance gene transfer mediated by a variety of mobile genetic elements may have largely contributed to the spreading of resistant strains of Shigella.

ABSTRACT

<p><b>OBJECTIVE</b>To establish objectively biomechanical criteria for temporomandibular joint disc tissue engineerings.</p><p><b>METHODS</b>Eight temporomandibular joint discs from a four-month-old goat were separated completely. A cylindrical sample with diameter of 3 mm in the anterior, intermediate, and posterior bands was obtained, and the samples underwent pre-compression test with three cycles under a speed of 0.025 mm · s⁻¹. With the end condition for the maximum load of less than 4.9 N and 1.5 mm compression displacement, the formal compression test was performed with the same speed. Moreover, this test obtained the stress-strain relationship and elastic modulus of each disc band. Ultrastructure and collagen fiber orientation of the district zone were observed by scanning electron microscope and polarized light microscope.</p><p><b>RESULTS</b>1) The physiological strain range of an articular disc was within 10%, and the quasilinear constitutive equation of articular disc tissues can be fitted with the polynomial function: y = ax + bx² + cx³. 2) The elastic modulus of the intermediate disc zone was the largest (P < 0.05). No significant difference existed between the anterior and posterior bands (P = 0.361). 3) Scanning electron microscope and polarized light microscope showed an annular disc collagen network structure, which was the internal part of the goat temporomandibular joint disc. The collagen arrangement of intermediate bands was tensely arranged anterior-posteriorly. The collagen of anterior and posterior bands went through mediolaterally with intersection of anterior-posterior branch fibersd.</p><p><b>CONCLUSION</b>Annular collagen network structure is the basis for goat temporomandibular joint disc compression resistance properties. The intermediate band demonstrated higher compression resistance performance than the anterior and posterior bands.</p>

ABSTRACT

Objective To observe the effects of Xifeng Tongnao capsules on the content of TNF-α and IL-1β in brain tissue of rats with focal cerebral ischemia-reperfusion injury. Methods A total of 120 adult SD rats were randomized into 6 groups: Xifeng Tongnao low-, middle- and high-dose groups, model control group, Buchang Naoxintong group and sham-operated group.Buchang Naoxintong group were treated with Buchang Naoxintong capsules at 0.864 g??kg-1.Xifeng Tongnao high-, middle-and low-dose groups were treated with 3.456, 1.728, and 0.864 g??kg-1 Xifeng Tongnao capsules, respectively;sham-operated group and model control group were treated with equal volume of purified water. Medications were administered intragastrically once daily for 7 days. The acute transient focal cerebral ischemia-reperfusion model was established by middle cerebral artery occlusion ( MCAO) 1 h after the final dose, and rats in the sham-operated group only received anesthesia and stripping without occlusion.All rats were sacrificed after reperfusion for 24 h, and expression levels of TNF-α and IL-1β in the brain tissue were detected by ELISA. Results TNF-αcontent in Xifeng Tongnao capsutes low-, middle-and high-dose groups were (35.34±8.95), (33.75±6.92), and (40.95±5.39) ng??L-1, respectively.IL-1β content were (1.44±0.47), (1.45± 0.23), and (1.61±0.33) ng??L-1 in low-, middle- and high-dose groups, respectively.TNF-α and IL-1β were (38.96±9.84) and (1.56±0.31) ng??L-1, respectively in Buchang Naoxintong group, (52.74±6.76) and (2.79±0.45) ng??L-1in the model control group, and (32.54±4.00) and (1.32±0.22) ng??L-1 in sham-operated group.TNF-αand IL-1βcontent were significantly higher in Buchang Naoxintong group than in sham-operated group ( P<0. 05 ) . TNF-α and IL-1β content were significantly decreased in Xifeng Tongnao high-, middle- and low-dose groups (all P<0.05). Conclusion Xifeng Tongnao capsules can protect brain tissue by supressing TNF-α and IL-1β and alleviating inflammatory injury from ischemia.

ABSTRACT

Objective To study the chemical constituents ofPronephrium triphyllum.Methods The chemical constituents in the plant were isolated and purified with silica gel and Sephadex LH-20.Their structures were identified by analyses of spectral data and physicochemical properties.Results Six compounds were isolated and identified as shelincaoide A(1),n-butyl-13-D-fructopyranoside(2),triphyllin A(3),6,7-di-hydroxycoumarin(4),daucosterol(5),and 13-sitosterol(6),respectively.Conclusion Compound 1 is found to be a new compound.Compounds 2 and 4 are firstly isolated from the plants in Pronephrium Presl.and all compotmds except 3 are obtained from the species for the first time.

ABSTRACT

Objective:To explore the clinical value of stereotactic wire-localized biopsy(SWLB)in digital mammography in the detection of microcalcification of the breast.Methods:A total of 45 patients with nonpalpable breast lesions which were positive for microcalcification by mammography but could not be detected clinically underwent SWLB.Their mammography fndings were analyzed in detail with pathology.Results:Among the 45 cases,13 cases(28.9%)had malignant lesions including ductal carcinoma in situ in 3 cases (20.1%),ductal carcinoma in situ with microinvasion in 4 cases(30.8%),invasive ductal carcinoma in 5 cases (38.5%)and intraductal papillary carcinoma in 1 case(7.7%).Thirty-two cases(71.1%)had benign lesions,2 cases(6.3%)of which were severe atypical hyperplasia.Conclusion:SWLB can accurately guide the surgical excision of nonpalpable breast microcalcification lesions and diagnose microcalcifications exactly,which is helpful for increasing the detection rate of eady-stage breast cancer.
